چکیده

Therapy [CPT]). All participants received treatment as usual in the form of CPT deemed appropriate for their presentation by the clinical physiotherapists and recorded using a standardised treatment schedule (1). The treatment schedule consists of a recording form and explanatory manual describing the treatment and this has been used successfully with clinical physiotherapists in our recently completed trials comparing CPT and functional strength training (FST) for the lower limb (2). The clinical physiotherapists providing CPT were trained to use the treatment schedule and documented content and amount of treatment provided each day. The records of treatment provided for all participants were collected by a researcher each week. The CPT as delivered in the centres for the present trial was a combination of hands-on techniques emphasising postural alignment and quality of movement together with goal-orientated task-specific training. Specific categories of physical therapy included: soft tissue mobilisation, facilitation of movement, splinting, resistive exercise, and functional mobility training. As in our earlier trial the purpose of the treatment schedule was not to constrain the clinical therapists in their choice of treatment for any individual (2-5). Clinical therapists were free to choose the techniques and the dose of treatment that they considered appropriate.

A Randomized Controlled Evaluation of the Efficacy of an Ankle-Foot Cast on Walking Recovery Early After Stroke: SWIFT Cast Trial.

BACKGROUND Timely provision of an ankle-foot orthosis (AFO) orthotist customized for individuals early after stroke can be problematic. OBJECTIVE To evaluate the efficacy of a therapist-made AFO (SWIFT Cast) for walking recovery. METHODS This was a randomized controlled, observer-blind trial. Participants (n = 105) were recruited 3 to 42 days poststroke. All received conventional physical t...

Design and Evaluation of an Articulated Ankle Foot Orthosis with Plantarflexion Resistance on the Gait: a Case Series of 2 Patients with Hemiplegia

Ankle-foot orthoses (AFOs) have been described to have positive effects on the gait biomechanics in stroke patients. The plantarflexion resistance of an AFO is considered important for hemiplegic patients, but the evidence is still limited. The purpose of this case series was to design and evaluate the immediate effect of an articulated AFO on kinematics and kinetics of lower-limb joints in str...

Evaluation of a Viscoelastic Ankle-Foot Prosthesis at Slow and Normal Walking Speeds on an Able-Bodied Subject

Objectives: This paper describes further improvement and preliminarily evaluation of a novel viscoelastic ankle-foot prosthesis prototype. The objective was to control the ankle hysteresis at slow and normal walking speeds. Methods: Inspired by the ankle biomechanics, in which the hysteresis differs based on the gait speeds, a manually damping control mechanism imbedded in the prosthesis for...

بررسی تأثیر ارتزهای مچ پا-پایی بر متغیرهای راه‌رفتن و تعادل افراد سکته مغزی: مطالعه مروری

Objective Stroke occurs when the supply of blood to the brain is either interrupted or reduced. The clinical presentation varies from minor neurological symptoms to severe deficits, depending on the location and the size of the brain lesion. Hemiparesis is one of the most striking features in the acute phase. Many other deficits may also be present, including postural imbalance. All persistent ...

Walking ability of stroke patients: efficacy of tibial nerve blocking and a polypropylene ankle-foot orthosis.

OBJECTIVE To investigate the efficacy of tibial nerve blocking by percutaneous radiofrequency thermocoagulation and an ankle-foot orthosis on the walking ability of stroke patients with a spastic equinus or equinovarus foot. DESIGN A placebo-controlled randomized clinical trial with a 2 x 2 factorial design and with a 3-month follow-up. SETTING Outpatient clinic of a department of rehabilit...

Effect of Rocker Bar Ankle Foot Orthosis on Functional Mobility in Post-Stroke Hemiplegic Patients: Timed Up and Go and Gait Speed Assessments

Objectives: Ankle Foot Orthoses (AFOs) are widely utilized to improve walking ability in hemiplegic patients. The present study aimed to evaluate the effect of Rocker bar Ankle Foot Orthosis (RAFO) on functional mobility in post-stroke hemiplegic patients. Methods: Fifteen hemiplegic patients (men and women) who were at least 6-months poststroke and able to walk without an assis...
